HIPAA Notice of Privacy Practices

This Notice applies to your Protected Health Information (PHI) that is handled by licensed medical providers who provide care to you through the Renoja Bio platform, facilitated by OlaMD, and by Renoja Equities LLC to the extent it functions as a business associate of those providers.

What Is Protected Health Information?

PHI is information about you — including your name, health history, lab results, diagnosis, treatment, and payment information — that can reasonably identify you and that relates to your health or healthcare. HIPAA gives you rights over your PHI and sets rules about how it may be used and disclosed.

How We May Use and Disclose Your PHI

Your PHI may be used or disclosed in the following ways without requiring your separate written authorization:

  • Treatment: Sharing your information with your treating provider, other members of your care team, or a specialist for purposes of evaluating and treating your condition.
  • Payment: Using your information to process billing and payment for services rendered.
  • Healthcare operations: Using your information for quality assessment, compliance activities, and program improvement.
  • As required by law: Disclosing information when required by federal or state law, court order, or law enforcement.
  • Public health and safety: Disclosing information to prevent or respond to a serious and imminent threat to health or safety.

All other uses and disclosures of your PHI require your written authorization. You may revoke an authorization at any time in writing, though revocation will not apply to actions already taken in reliance on that authorization.

Your Rights Under HIPAA

You have the right to:

  • Access your PHI: Request a copy of the health information we maintain about you.
  • Request corrections: Ask us to correct PHI you believe is inaccurate or incomplete.
  • Request restrictions: Ask us to limit how your PHI is used or disclosed (we are not always required to agree, but will consider all requests).
  • Request confidential communications: Ask that we contact you in a specific way or at a specific location.
  • Receive an accounting of disclosures: Request a list of instances where your PHI was disclosed other than for treatment, payment, or operations.
  • File a complaint: If you believe your privacy rights have been violated, you have the right to file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr/privacy. You will not be penalized for filing a complaint.

By Agreeing to This Consent, You Understand and Acknowledge That:

  • You consent to receiving healthcare services through telehealth technology.
  • You have the right to withhold or withdraw your consent to telehealth services at any time without affecting your right to seek care through other means.
  • You may suspend or terminate your participation in Renoja Bio services at any time.
  • In a medical emergency, you will call 911 immediately. Renoja Bio and its provider partners cannot connect you to emergency services.
  • Alternatives to telehealth, including in-person care, are available to you.
  • Your provider may consult with other licensed physicians regarding your care. All such consultations are conducted under obligations of confidentiality.
  • Technical failures are possible and outside the control of Renoja Bio and its provider partners. You agree to hold Renoja Bio and OlaMD harmless for delays or information loss due to technical failures beyond their reasonable control.
  • You are responsible for providing accurate, complete, and current health information. Providing false or incomplete information may affect the safety and appropriateness of your care.
  • You are responsible for maintaining the technology (device, internet connection, email access) necessary to participate in telehealth services.

How We Protect Your PHI

We implement appropriate administrative, physical, and technical safeguards to protect your PHI. Our telehealth partner (OlaMD) and pharmacy partner (RxAve) are bound by Business Associate Agreements that require them to protect your PHI in accordance with HIPAA.

Changes to This Notice

We reserve the right to update this Notice. We will post the current version on our website. The updated Notice will apply to all PHI we maintain, including information collected before the update.
