Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") governs your use of telehealth services provided by licensed medical providers facilitated through the Renoja Bio platform. Please read this document carefully before completing your health assessment or enrolling in a program.

By checking the consent box during enrollment, you confirm that you have read, understood, and agree to the terms of this Consent.

What Is Telehealth?

Telehealth is the delivery of healthcare services using electronic communications — including secure video, asynchronous messaging, and digital health assessments — between a patient and a licensed medical provider who are not in the same physical location. Renoja Bio uses telehealth technology to connect you with licensed physicians and medical directors who will review your health information and, where appropriate, recommend and prescribe treatment.

Your Providers

Medical services through Renoja Bio are provided by licensed physicians and medical directors through our telehealth partner, OlaMD. Prescription medications, where applicable, are fulfilled through RxAve, a licensed compounding pharmacy. The identity of your specific treating provider will be disclosed to you through the patient portal.

Renoja Bio (Renoja Equities LLC) is the program platform. It is not your medical provider and does not make clinical decisions.

Potential Benefits of Telehealth

  • Access to physician-guided care without in-person office visits
  • Faster enrollment and program initiation
  • Convenient monitoring and follow-up through your patient portal
  • Access to care regardless of geographic location (subject to provider licensing by state)

Potential Risks of Telehealth

  • Technical failures — including internet outages, platform issues, or device malfunctions — may delay your consultation or communication with your provider
  • In rare cases, information transmitted digitally may be of insufficient quality for clinical decision-making, requiring a repeat submission or in-person visit
  • Your provider may determine that your condition requires in-person evaluation and will refer you accordingly
  • Telehealth does not replace your primary care provider; you are responsible for coordinating your Renoja Bio care with your existing healthcare relationships

By Agreeing to This Consent, You Understand and Acknowledge That:

  • You consent to receiving healthcare services through telehealth technology.
  • You have the right to withhold or withdraw your consent to telehealth services at any time without affecting your right to seek care through other means.
  • You may suspend or terminate your participation in Renoja Bio services at any time.
  • In a medical emergency, you will call 911 immediately. Renoja Bio and its provider partners cannot connect you to emergency services.
  • Alternatives to telehealth, including in-person care, are available to you.
  • Your provider may consult with other licensed physicians regarding your care. All such consultations are conducted under obligations of confidentiality.
  • Technical failures are possible and outside the control of Renoja Bio and its provider partners. You agree to hold Renoja Bio and OlaMD harmless for delays or information loss due to technical failures beyond their reasonable control.
  • You are responsible for providing accurate, complete, and current health information. Providing false or incomplete information may affect the safety and appropriateness of your care.
  • You are responsible for maintaining the technology (device, internet connection, email access) necessary to participate in telehealth services.

Privacy and Confidentiality

Your health information will be protected in accordance with HIPAA and applicable state law. Please review our HIPAA Notice of Privacy Practices for full details. When participating in a telehealth consultation, please ensure you are in a private location where your conversation cannot be overheard by unauthorized individuals.

Compounded Medications

If your provider prescribes a compounded medication through RxAve, you acknowledge that compounded medications are not FDA-approved as finished drug products. Your provider will discuss the risks, benefits, and alternatives before prescribing. You consent to receiving compounded medications where clinically appropriate and ordered by your provider.

State-Specific Limitations

Telehealth services are subject to the licensing laws of the state in which you are located at the time of your consultation. Not all programs or medications may be available in all states. If services are unavailable in your state, we will notify you and issue a full refund of any program fees paid.

Withdrawal of Consent

You may withdraw this consent at any time by contacting us at [email protected]. Withdrawal of consent will result in the discontinuation of telehealth-based services through Renoja Bio. Any clinical records generated prior to withdrawal will be retained in accordance with applicable law.
